– Location provides tailored medical care to seniors from a medical team specializing in aging, chronic conditions – Murrieta, Calif. (Nov. 1, 2024) – Neighborhood Healthcare, a regional Federally Qualified Health Center providing a wide range of medical, dental, and behavioral health services, announced today the November 1 opening of its second Program of All-inclusive Care for the Elderly (PACE) center.
PACE is designed to deliver comprehensive and coordinated care to older adults with chronic health conditions. By integrating medical, social, and support services, PACE allows seniors to receive personalized care in their homes and communities rather than in nursing facilities. This approach empowers seniors to maintain their independence while ensuring access to essential health services tailored to their needs.
The nearly 20,000-square-foot facility in Murrieta will offer tailored medical care to seniors from a medical team specializing in aging and chronic conditions.
For many, the facility offers a welcome alternative to a nursing home.
“Community health is about more than just vaccines and checkups. It’s about giving people the resources they need to live their best lives,” said James Schultz, MD, Chief Medical Officer of Neighborhood Healthcare. “Caring for seniors in this manner means providing the resources to do so comprehensively, empowering these individuals to thrive in their community. We’re excited to open this facility in Murrieta!”
“PACE is all about heart and community,” said Rashid Altafi, MD, PACE Medical Director of Neighborhood Healthcare. “Our dedicated team members make a profound difference by building meaningful connections with each participant, addressing their unique needs every day. Their compassion, understanding, and teamwork enable our seniors to remain in their homes while enjoying a high quality of life. At the core of our mission is a commitment to providing the extra care and support that ensures every senior feels seen, valued, and empowered as they age.”
Neighborhood Healthcare’s bilingual medical team works with therapists, dieticians, social workers, and home care nurses to create a customized care plan for each participant to ensure optimal health, safety and well-being. The facility is located at 30135 Technology Dr, Ste 100-150, Murrieta, CA 92563.

To Our Valued Donors, Sponsors, and Community Partners,
Individuals, families, and philanthropic organizations have shown their generosity by donating money, food, and supplies to support Neighborhood Healthcare and our most at-risk patients, the uninsured. The outpouring of support is felt by each donation, each message, each piece of Personal Protective Equipment (PPE), and each smile. And be it one mask or one dollar, you play the most important role in helping us care for our critically ill patients who, without donations, may not have access to Neighborhood’s life-saving care.
This year has proved challenging although with each challenge came an opportunity. For example, to continue providing access to patients during COVID-19, we transitioned our patients to telehealth and video visits, a service that was unavailable pre-pandemic. To transition thousands of patients to a virtual platform took time, but within weeks we saw less no shows, improved patient and employee satisfaction.
More so, we had high-risk patients confined to their homes and realized their barriers to care were debilitating. In response, we created the Mobile Health Team to serve qualifying patients at their homes throughout both San Diego and Riverside counties. Despite the unprecedented times, we continued to expand services, providing everything from pediatrics to senior care – none of which is possible without the support of our thoughtful donors, sponsors, and community partners.
In addition, we opened a new site in Temecula dedicated to the teaching of future physicians. In partnership with United Health Services (UHS), we have a Family Residency Program where our physicians will teach medical care, but more importantly, medical care that is culturally sensitive and community based. To work alongside UHS and new residents is a gift for both Neighborhood and our patients.
Most recently, we broke ground in Riverside and we will soon open two new healthcare facilities: care for the elderly and a Federally Qualified Health Center (FQHC). We will be expanding our mission and our promise to a new neighborhood because we believe that a community is only as healthy as its residents and because people like you, who courageously support our mission, make it possible.
Regardless of where we are or where we go, we are better together.
Thank you for your unwavering support – we couldn’t do it without you.
Gratefully yours,
Dr. Rakesh Patel
