This event features:

• • PRACTICAL SOLUTIONS: Healthcare and Community Well-Being – This panel will focus on the hope, dignity, and practical solutions that emerge when systems work together with compassion. Through Neighborhood Healthcare’s Behavioral Health Services and One Safe Place, attendees will learn how trauma-informed care, trusted partnerships, and accessible services can help vulnerable individuals and families find support, healing, and a path forward.
• • AGING WELL: Innovative Approaches to Healthy Longevity – This panel will highlight Neighborhood Healthcare’s Senior Oral Health Program, the San Diego Senior Community Foundation’s investment in senior well-being and emerging Longevity Centers, and Neighborhood Healthcare’s PACE model for coordinated, whole-person care. Together, these programs offer practical examples of how philanthropy, healthcare, and community partners can reimagine aging services, improve access, and support healthy longevity for local seniors.
• • BETTER TOGETHER: Partnerships that Strengthen Community Health – Learn how three distinct and different programs are building healthier communities by working across sectors, sharing resources, and meeting people where they are. From student basic needs and workforce pathways to whole-child community schools and innovative health, education, and wellness partnerships, this panel will highlight practical ways collaboration can expand access, improve well-being, and spark new ideas for partnerships that strengthen the entire community.
